Customer-supplied materials sent to Star Gems must include a completed, printed Incoming Materials Form. Missing or incomplete information may delay your order while our team identifies the materials and confirms how they should be handled.

Complete this form whenever you're sending customer-supplied materials to Star Gems, including:

Loose stones

Mounted stones

Scrap metal

Scrap metal containing stones

The form also tells our team whether materials should be reused, returned, or credited as part of your project.

Not sending customer-supplied materials? You don't need this form for orders where Star Gems is supplying all materials.
